Root Cause Analysis (RCA) gets to the bottom of an issue so that action can be taken to fix it permanently. By systematically peeling away layers of causes and effects, RCA identifies breakdowns in processes and systems that contributed to the event. Done correctly, RCA will improve safety, reduce future incidents – and ultimately lower your total cost of risk.

The participant will learn the RCA methodology with a series of group work, role-play and simulation on real case RCAs.

2.Collect relevant data required data for the Root Cause Analysis

3.Organize and  analyze the information gathered during the investigation and identify gaps and deficiencies
