A root cause is a factor that caused a nonconformance and should be permanently eliminated through process improvement.

Root cause analysis is a collective term that describes a wide range of approaches, tools, and techniques used to uncover the causes of problems.

Some root cause analysis approaches are geared more toward identifying true root causes than others;

Some are more general problem-solving techniques, while others simply offer support for the core activity of root cause analysis.

RCA is a process designed for use in investing and categorizing the main cause of events with safety, health, environmental, quality, reliability, and production impact.

Root Cause Analysis (RCA) gets to the bottom of an issue so that action can be taken to fix it permanently. By systematically peeling away layers of causes and effects, RCA identifies breakdowns in processes and systems that contributed to the event. Done correctly, RCA will improve safety, reduce future incidents – and ultimately lower your total cost of risk.

The participant will learn the RCA methodology with a series of group work, role-play, and simulation on real case RCAs.

This session will help the participants to

  • Instigate and determine why an event occurred in a given circumstance
  • Collect relevant data required data for the Root Cause Analysis
  • Organize and  analyze the information gathered during the investigation and identify gaps and deficiencies
  • Map the root cause to identify the underlying reason for each factor
  • Develop effective countermeasure and recommendations
  • Make an Action plan, execute and monitor the progress of implementation
